Ok, I emailed a couple of local therapists and two have responded:

Thanks for your e-mail and interest in gender information. I
would be happy to see you..I see people for $150.00 for l l/2 hour. I do not
take insurance directly but can give you a receipt with a mental health code
that they may reimburse a percentage directly to you. I have offices in
Coral Gables and Hollywood. The other option is Dr. Carol Clark in North
Miami who may take insurance and she also has a group for TG/CD's..Her
number is XXX-XXX-XXXX...I think it is VERY good to see someone who
specializes in this field as well as a support group for you and your S.O.
when you choose to disclose.

At this time, I don't take Aetna insurance, but I do have a sliding scale to help folks out. Paying privately keeps things completely confidential, which you indicated is a concern. All of my files are completely private. Of course my home is private and even in my office, the secretary does not have access to my files.
