surprised by Mass DPH gender choices

I was hanging out with some friends at a local social services agency, and the topic of discussion was the state paperwork from the Massachusetts Department of Public Health. And being in this somewhat genderless state myself, currently, I wasn't expecting to fit in to the DPH-supplied gender choices. So, I asked, "How many gender choices are there?" and was very surprised by the answer:

• Male


• Female


• Transgender


• MtF


• FtM


• Unknown

I guess DPH is really trying to be as inclusive as possible. I'm really glad they included transgender, otherwise, none of the other 4 choices would have applied. How does everyone else feel about transgender being the choice for people who are "both" or "neither"?
