Hey all, still doing research about having an orchiectomy, the material I keep finding, which is so minimal and pretty much limited to folks with experiences of "i'm having an orch and plan for vaginoplasty in the future" doesn't represent my personal experience with my body and needs around transition. I guess I'm curious if there are folks here who had an orchiectomy and are into their penises, or kinda into them, and where your desire for this surgery comes in.

Also, the material I keep finding hasn't ever really mentioned anything about what happens to individual's scrotal tissue in/after the procedure.

I'm not particularly interested in the aesthetic of a scrotal tissue removal along with the orchiectomy, that's my personal preference though. I curious if folks have had surgery, kept their scrotal tissue (for whatever reason, i'd like to hear if you'd share), and essentially what it looks like. I guess I can imagine a lot of things; the tissue retracting and the scrotum/lack-of-testicles looking small (this is actually what I'm after), the tissue looking just like extra skin, etc.

Obviously everyone is different, but this lack of information feels really frustrating as it seems like my desire around transition, surgery, my body is not accessibly represented anywhere, and I guess I'm just calling out to folks to share if any of this resonates with them!

hey all, i've been poking around the internet for resources in the bay area (i'm in sf) about local-ish doctors who do orchiectomies with really no luck. Seems like everyone's down for Kimmel, but frankly, it would be much easier to get to come home to recover vs. being across the country. Thoughts? Experiences? Thanks!

For some reason, my dermatologist has chosen to put me back on anti-androgens, three and a half years to the day after I had my SRS, which is something that should have effectively negated any production of Demon T within my body, the production centres being sent for destruction on that day.

I understand the effects of Demon T on hair growth and and all, but this guy seems to have questions about why my face is so 'hirsute', as he puts it, after those three and a half years. Does he not understand that, once the Demon T has had its effects on the human body, certain things cannot and will not disappear without certain interventions, of which electrolysis is one? I've been doing electro for a year now, and had a fair number of laser sessions before that, and there's no doubt it's doing the job. My cheeks are pretty much free of the terminal hairs that it's supposed to get rid of. Ditto my upper lip. I really do not see any point in taking this stuff.

I'm quite happy to take the scrip he wrote me for Androcur and get it filled at the pharmacy on base (for free), then just not take it. I know a number of people who would gladly take it off my hands, since the stuff is expensive...my partner for one.

So, is it just me or do I have an asshat for a dermo? Should I be taking this stuff even though electro's doing it's thing nicely?

Now that I have had an orchiectomy, I have two different courses my HRT could go.

Estradiol and Medroxyprogesterone dosages will stay the same.
I am specifically referring to Spironolactone -- that wonderful diuretic anti-androgen.

I recognize that with removal of the testicles, I've eliminated the Major source of male sexual hormones. But I know that there is still a small amount of testosterone that is produced by other organs within the body.

My doctor for HRT is Dr. Horowitz at Cedar-Sinai in Los Angeles. I pay out of pocket for him (outside of my medical coverage). He is specialized in trans* patients and is extremely well known for this. I still have to talk to him about his distinct recommendations on medication changes, but from previous discussions with him... I am under the impression that Spironolactone is no longer needed AT ALL because the amount of Estradiol in the body will suppress any testosterone production so that it's insignificant in my overall schema.

But I also hear from other sources that a small amount of Spironolactone is still necessary to block out that itty bitty little bit still being produced. In fact, the endocrinologist under my current medical plan that I saw briefly before surgery recommends this.

The major reason I got the surgery was specifically to be able to cut down or completely eliminate Spironolactone. My stamina was so low that I'd get winded going up one flight of stairs. It was uncomfortable basically being dehydrated for the last two+ years.

Dr. Horowitz has explained that even though I have been taking an anti-androgen, the pituitary gland up in the brain still knew that my testicles existed. But once those are removed, and after it gets over it's initial shock, it will start to look for ovaries... and will find those in the shots I am giving myself. This aligns with cases I've heard of some transwomen having growth spurts after surgery. This is the secondary (and more of a "fingers-crossed") reason I got the procedure done.

Technically, my body is still going through puberty. I am afraid of impeding on that puberty by not suppressing additional testosterone production... but still wondering if it's necessary to do. I am almost two and a half years since starting HRT... and my breast development hasn't been exactly spectacular. We'll see how it all goes, but I have a few questions on my mind for those reading this that have personal Orchiectomy experience or have studied it.

1) How old are you?
2) How long have you been on HRT? ..and do you self-administer or do you see a professional for it?
2) After surgery... did you eliminate completely or simply cut down your intake of anti-androgens?
3) After surgery... Did you notice any secondary sex characteristics (breast development, etc.) start to develop more or was progress exactly the same as before?

...and for those that had been on a high dosage of Spironolactone... and completely cut it out after surgery...
4) How long until you started to feel new energy and new stamina and new life (due to cutting Sprio?)

Gosh... I know there are other things I want to ask, but really can't think of any more at the moment.

I know that if I was completely past any sort of physical development stage... I wouldn't care about that little bit of testosterone still being produced. But right now, I do want to hopefully see more physical changes in myself. Thank you for helping me out with your answers!!

This may seem like a slightly odd question, but I'm considering seeking an endo out to ask about the possibility of supplementing my testosterone level. I'm a year post-op, and although everything is sensate and works downstairs, I basically have no significant libido. This doesn't bother me overly - it's actually quite peaceful, but I think my wife would like me to show a bit more interest at times, and I do feel I'm missing out a bit.

So, my question to other post-ops: is anyone on testosterone supplements for this (or any other) reason? How do you find them? If they're just to take one up into the normal female range, do they cause problems with facial and body hair regrowth? Any other issues I should be aware of?

OK this isn't a poll but I wanted to ask about people who have had orchiectomies. Where did you have your surgeries. I've seen some post on here and on the MTF community about a specific doctor that some people would not go to. At any rate kinda same rules as the polls, please comment if you have had a surgery, not trying to limit this. I guess you can talk about the doctor you would like to go to and i know there are some people who have some surgeries planned in the near future. The idea of all these posts is to figure out which doctors are the most popular and also to find out names of doctors around the country.

I have seen postings on here from others stating that they would not go to Dr Kimmel for their Orchiectomy. I am strongly considering it. For those of you who have read or know of any negative experiences with Doctor Kimmel, would you please share with me? (links, first hand, etc) I have only heard positive experiences and would like to know the negative as well. Thanks!!!

I want to get an orchiectomy within the next few months, but I'm really not sure to start. I'm having trouble finding "how-to" resources on the steps I'll need to take as a trans woman; just a lot of things telling me how I would do it if I have testicular cancer and therefore doctors who won't be so reluctant to do it so that I'll need to do a lot of running around to a lot of different people asking for permission slips or things debating whether or not orchiectomies are a good choice for trans women but not actually saying how to go about getting one.

Halp me Dr. LJ!

Also, I'm in the Albany, NY area, so any information about resources within like a 100-mile radius of here would be greatly appreciated.

After reading qualdy's post below, I thought I'd post to see if anyone has experienced going off estrogen after having an orchi or GRS.  I would think the changes would be minimal, since T isn't in the equation anymore, but would like to hear from someone who's experienced it.

ok, first off, i dont have an orchi scheduled, or i'd be able to ask someone all these questiosn, for now all i have is a very very vague letter telling me my latest blood tests show some abnormalities with my T level, and that i've to get some tests done, may need to stop my AA or may need some "minor surgery" (i've filled in blanks with, "your nuts dont work, we dont knwo why, we may cut them off and then we'll at least knwo why")

what i'm really looking to know is: how long wil i need to take time off from school / college (depending on your wording, it's further education, a childcare course) will i be okay to go back after a day or two, even if i have to take a cushion?

how long until i can realistically run around after a room full of preschoolers for 8 hours

and how long until i can lift 10+ kilo boxes for 8+ hours?

thanks for listening, and thanks for any advice, just dont like this whole not knowing thing, so am trying to arm myself with info.

I have a question about getting an Orchiectomy. My doctor has recommended this and knows that this is as far as I can go given my MS. Lots of issues and going for GRS is not available to me.

My Neuero wants me to get off or reduce my HRT given the cocktail of MS drugs and I have to be careful.

So This is an option that I need to look at seriously. What I want to know is how much sexual activity can I expect if I have this procedure? Does anyone have experience with an Orchiectomy procedure and what have you experienced.

The HRT has been great but one of my MS meds has some nasty side affects with the HRT meds and I have to chose.

I've been thinking a lot lately about how realistically no doctor on earth is going to put me forwards for SRS. With my heart condition it's just not an option. However I was reading up on alternatives and a castration or orchi(d)ectomy (people seem to disagree over the correct term) will almost certainly be beneficial, reducing the amount of medication I need and such.

To be perfectly honest if I could, I'd have this done right now., but you know, proecedure, etc. Bu i can still read up.

Anyway, I was looking at this webiste and I found this image:

I was wondering if anyone with more knowledge of the procedure could tell me if that was a pre-SRS picture or not? Because there just don't look like there's enough left for it to have only been an orchi', and the caption suggests that there were difficulties in sugery which might have left that...

The reason I ask is that I sort of have my fingers crossed that if that kind of shrinkage of the genitals is normal after an orchi, I don't feel so bad about probably not being able to have full SRS....

Also I hear the SOC says nothing about when an orchi can be had in transition, does anyone know more about that? I'm pretty resolved to the fact that as soon as I can afford it I'm going private now so I'd like to know when to raise the question to the doctors.
