Sex life post orchi-slight problem

Hi, I'm a 32 year old trans woman who got an orchi last spring. Its all healed up, and I'm enjoying the changes. However, I've noticed in the past month or so that I've been having some painful erections when I try and masturbate. I never had this problem prior to surgery and taking just hormones (a very low dose of 2mg a day) and spiro( also a low dose of 100 mg a day). I'm now taking 2mg a day of oral estradiol.

I start masturbating, and...about 30 seconds later my erection becomes rather painful. My surgical site is all healed up, and there are no complications there. I masturbated several times in the first couple of months after surgery, but lately I haven't because of this rather annoying issue. Is this normal?
