GLBTQIA Books for School and Public Libraries?

I'm making a list of recommended GLBTQIA (gay, lesbian, bisexual, transgender, queer, questioning, intersex, ally) books to have in an elementary school library, a middle school library, a high school library, and a public library... what books do you recommend for these various libraries?

I'm specifically having trouble finding "age-appropriate" books for elementary and middle-schoolers... but any input and any book suggestions are welcome; just tell me what sort of libraries you think they're good for.

(Please let me know if you've read the books you're recommending).
