North West Spain and Travel Insurance

transA couple of related questions:
Anyone know what North West Spain is like in terms of TS issues? (Specifically, Santiago de Compostella and the surrounding area) I'd assumed I'd have no problems visiting for a week but having read about some recent stuff in Turkey I'm concerned what might happen if I'm not passing. (I know, Turkey is highly Muslim and Spain is Catholic, but it still worries me) Generally I pass "ish" but certainly not all the time.
Secondly, can anyone recommend a travel insurance company that will cover me for a week in Europe and not ask stupid questions about a pre-op MtF? (I can't really hide it because they ask things like "When did you last see a doctor"/"What prescription medication are you taking")
