Skipping the standards of care?

I've been looking at some transsexual resources online, and while they're all wonderfully informative the one thing that seems to be lacking is how to skip going through the standard of care if you want to transition. Is this just a matter of finding an endocrinologist who'll agree to monitor me without a letter (probably not an easy task), or is there some other angle I could work? I'm not much interested in self medicating, and I'm not on anything right now.

Just don't want to deal with therapists.
