Trans-inclusive health insurance?

Does anyone know of a model trans-inclusive health insurance policy for colleges/universities?

I am writing recomendations as to how the health insurance policy at my school ought to be revised to be more inclusive of GLBTQ ppl, but I'd rather not re-invent the wheel.

What schools have good/model insurance policies? Or for that matter, any insurance ploicy-- not just for schools-- that could serve as a model?
