Need help with a letter for my doctor

My therapist is finally going to give me the letter to give to my doctor but she has no idea what to write in it. I remember there are key words and phrases that need to be in there and have had letters saved in the past just for this occassion but have no idea where they disappeared to.

Any help would be appreciative. She needs this email sent tonight or tomorrow.

Also to add to this.. Has anyone had experience with going from self medicating to doing it legally? Should I tell my doc I am already on meds and have been medicating since 8-22? What if he gets upset with me? I am concerned that he will drop my dosages and I have been there when I was running out of meds. Dropping down my dosages = hell. Also what are the chances I can talk him into putting me on injectables? I have issues taking pills and would especially like the convenience of only having to inject once a week vs putting pills under my tongue for 30 minutes twice a day (especially since i will be moving in with 2 guys and might not get the privacy i need during those two 30 minute sessions.
